Effective 1/5/04 any student requiring a sports physical must use a specific form developed by the New Jersey Department of Education. This form must be filled out by the student's parents and family physician or by the school physician Dr. Raab in order for any student to participate in school sports. No other physical form or note from a physician can be accepted. Any student who has already had a sports physical this year (from 8/03) will not need a new physical for this year. This new law only concerns those students who wish to participate in spring sports who have not yet had a physical. This new form must be used from this point on for any sports physicals- for next year and beyond. Please see me with any questions and for the forms.

Health Screenings will be done according to State regulations as follows:
Hearing: grades Pre-K,K,1,2,3,4,6,8
Vision: grades Pre-K,K,2,4,6,8
Scoliosis: 4,6,8
Physical Examinations are required for all new students and recommended for 4th & 7th grades.
